The CD has not been for sale since about 2010 and that arrangement ended years earlier, so the form below is a record of how ordering once worked and not a way to order anything. Do not post money, and do not write your card details into it — nobody is receiving them, and the address may since have passed to someone else.

The price for one CD is US $15.00. The shipping and handling fee is a flat US $4.00 (independent of the number of CDs ordered). The easiest will be, if you print out this page, and fill out the following order form:
